Lexington Family Physicians, P.A. Payment and General Patient Information
Lexington Family Physicians, P.A. has set its fees at the usual and customary rates for family practice in this area. The complexity of your problem determines our fees. We offer discounted rates for eligible families, ensuring access to quality care for all. We will not deny access to services due to one's inability to pay. Discounted rates are available for qualified applicants. Contact us for more detailed information prior to your visit.
We request your insurance information before your physician sees you. Please let us know if your insurance coverage changes before your next office visit. Your insurance policy is a contract between you and your insurance company.
If you have any issues, we will do what we can to assist you in obtaining the full benefits of your insurance coverage. Deductibles, co-payments, and other balances not covered by your insurance carrier are your responsibility.
Upon discharge, we will estimate your deductible and co-payment so that you may pay at that time. Again, if you have any questions regarding our fees, we encourage you to contact us.

Other Fees
Periodically you may need special services from our office and there’s a nominal charge to cover these extra costs.
We charge for the following services:
- Returned checks
- Requested copies of the medical chart by your insurance company, attorney
- Transfer your records to another practice.

Liabilities
The patient's responsibility, the person receiving care, is to pay for services rendered promptly. Therefore, a liability suit against someone else is not a reason for delaying payment to us. Once your account has been financially satisfied, we will assist you by providing your attorney with the necessary medical information. The patient must sign a "release of information" form accompanying any such requests.

Workers’ Compensation
All patients who present themselves for treatment due to a work-related accident (worker's compensation) must have their employer provide the necessary paperwork. Should the workers' compensation carrier deny the claim, it is the patient's responsibility for payment.

Medical Care
Bring all medicines which you are taking to each appointment. We expect you to provide us with your medical history and information about any care you obtain outside of our practice, including your current medications, recent test results, visits to other doctors and healthcare providers, behavioral health services, hospitalizations, and emergency department visits.

Preventative Care
Your Preventative Care Visit (Physical) includes:
- A routine physical exam
- Immunizations
- Services defined by the Patient Protection and Affordable Care Act
Most insurance plans no longer charge a coinsurance (copay) or deductible for preventative services. If you have other health issues, your insurance may require Lexington Family Physicians to charge a copay for that visit. Check with your insurance company about what your plan covers preventative services. If you discuss symptoms of acute or chronic diseases at your Preventative Care Visit (Physical), it is considered "diagnostic," and you will most likely be required to pay a copayment.
